Insurance Verification
Verify Your Coverage
Fill out the form below and we'll confirm your insurance benefits before your visit. We only use this information to check your coverage.

How insurance verification works
Send the form
Tell us your name, how to reach you and your insurance plan. You can add your date of birth too, which helps us look up your plan. If you have a question about your coverage, add it in the notes.
We check your benefits
Our team checks what your plan covers for your visit: the exam, frame allowance, lenses and contact lenses, so there are no billing surprises at the front desk.
We get back to you
We contact you by phone or email, whichever you chose, within one business day, to confirm your benefits before your appointment.
Prefer not to use the form? Call us at (718) 998-8400 and we'll look up your benefits, or book through Zocdoc.
What to bring to your visit
- Your insurance card (Medicaid card, or your Medicare or Medicare Advantage plan card, if that is your coverage).
- A photo ID.
- Your current glasses or contact lenses, and your prescription if you have one.
- If you wear contacts, the box, or a note of the brand and prescription numbers.
Using Medicaid or Medicare? See what to bring and what each covers on our Medicaid and Medicare pages. Medicare Part B covers medically necessary eye exams, such as exams for diabetic retinopathy or other eye conditions; routine refraction exams are not covered by Part B.
